About the program

When it comes to metabolic disease, lifestyle changes can be powerful medicine. But changing long-established behaviors on your own can be a daunting challenge. That’s where our metabolic health group program comes in. Whether you have hypertension, pre-diabetes/diabetes, high cholesterol, obesity, or metabolic syndrome, we’ll help you take control of your metabolic disease by making therapeutic lifestyle changes you can stick to.

What to expect

Led by One Medical providers and health coaches, this program consists of six 2-hour sessions (meeting every other week). We’ll provide evidence-based resources and information on diet and exercise changes, and teach you effective strategies to enable, advance, and maintain these changes to improve your metabolic and cardiovascular health.

Our community-based approach focuses on coaching to change behaviors and troubleshoot challenges — with the added benefit of peer-to-peer learning and support.

The topics covered include:

  • Goal setting and encouraging a growth-oriented mindset
  • Eating a balanced, whole-foods, plant-based diet
  • Getting regular exercise
  • Stress management
  • Improving sleep
  • Self-monitoring of lifestyle and biometrics
