PPO, HMO, EPO? It gets complicated, we know. In most cases, your plan type is listed on the front of your insurance card. Can't find it? Just give your insurance carrier a call and they'll be able to help. If you have an HSA or FSA, you may be able to be reimbursed for your membership fee payment.
