Medical Records

For Amazon One Medical members

One Medical members can access and download their medical records at any time in the One Medical app or website (click here to login). Records can be saved for your personal use or shared with other healthcare providers.

  • In the One Medical app: Sign in → go to My Health → select Get My Records
  • On the One Medical website: Sign in → click on your initials (upper right corner) → select Account Settings → select Get my Records

For Care Partners

(Care partners include providers, specialists, lab facilities, pharmacies, Health Insurance, regulatory agencies, and others.)

All medical records requests from care partners must be submitted by fax or email. Please provide a patient-signed medical records release form and clearly indicate the records you require, or have the patient complete the Amazon One Medical Records Release form (English or Spanish). All submissions must include all required information or processing may be delayed. All requests must include the appropriate authorization (e.g., patient consent form, affidavit,etc.).

Processing timelines:

  • Requests are processed as quickly as possible in the order they are received.
  • Standard requests are typically fulfilled within 15 business days, though up to 30 business days may be required per HIPAA guidelines.
  • Large requests will take longer to process.
  • Requests can be marked as urgent if the patient has an emergency need for care. Requests that are marked urgent that are not in support of an emergency will be processed as a standard request with the delivery timeline outlined above.
  • Follow-up inquiries can be emailed to medicalrecords@onemedical.com and are handled as quickly as possible, but please note that our highest priority is on processing requests.
