1. Should I sign up now or wait till I get sick?

   We recommend establishing care now, so that our team can get to know you and better care for you if something comes up. In addition, once you become a member, you’ll be able to access a provider 24/7 anytime, anywhere. Our Virtual Medical team is always available if you need advice in the middle of the night, and as a member, you can also use our mobile app to request treatment for common issues. Activate Now.

2. Dartmouth offers the One Medical benefit to my spouse, and adult dependents (18+), how do they register?

   Spouses and dependents can follow the same activation instructions as you did. Your spouse and/or dependents will need to register as individuals, which creates their personal medical charts. You can help your eligible dependents set up their account, but they will need to accept the terms of service themselves.

3. What happens to my membership if I leave Dartmouth?

   The Dartmouth sponsored membership is only available to employees of Dartmouth, their spouse and adult dependents (18+), and Dartmouth retirees enrolled in traditional Medicare A & B and the Dartmouth Medicare Supplemental plan (DCMS). If you leave Dartmouth, you’ll receive an email from One Medical alerting you that your Dartmouth-sponsored membership will be ending.

4. Who has access to my health information?

   One Medical at Dartmouth will use and disclose your personal health information in accordance with HIPAA and other applicable privacy laws. Providers who treat you will typically have access to your medical records. For more about our HIPAA privacy practices, please see our HIPAA Notice.

   One Medical takes your privacy very seriously and we are committed to protect the personal information that you share with us.

5. Am I charged for lab tests at One Medical?

   There is a lab at One Medical at Dartmouth for your convenience. With this, we are able to perform blood draws and some lab tests onsite. If lab work is sent externally, copays and deductibles may apply. Your care team can help you understand the process prior to doing any tests.

6. How do I transfer records?

   If you wish to leave the One Medical at Dartmouth practice, please follow these two steps:

   • Log into the One Medical app, click on My Health, select Get My Records, and follow the prompts to have your medical records transferred to your new provider
   • Email dartmouth@onemedical.com with your name and date of birth and ask to deactivate your account.

   If you have a copy of your records you'd like to send to One Medical:

   • Attach the records to a secure message in your One Medical account or
   • Fax records to 602-218-4076 or
   • Email records to medicalrecords@onemedical.com
