The opportunity

We are seeking a full-time Physician leader to join our growing, outpatient practice in Glendale, AZ.

 

The Care Team Medical Director will work closely with the Market Medical Director over the Phoenix market to play a critical role in both care delivery and ongoing practice innovation. Rediscover the "joy in practice" while working in a value-based care environment with a team to support you in delivering high quality patient care.

Our practices offer smaller panel sizes, no billing or coding, and the opportunity to lead systemic change in health care delivery while working with a true team. What you'll likely work on:

  • Live our mission of “restoring humanity to healthcare”, for patients and for yourself.
  • Responsible, in partnership with the Clinical Team Manager, for ensuring that the practice team functions effectively as a unit
  • Manage and develop the care team (Nurse Practitioner, Physician, Behavioral Health Specialist) understanding both their current performance and long-term professional goals
  • Serve as visible team leader; solve problems, make sound decisions; help others with sound decision making, empower teams to work at and achieve their best
  • Ensure the practice adheres to evidence-based practice and engage patients in care
  • Ground care in patients’ self-identified values; practice active shared decision making and support advanced care planning
  • Drive quality (e.g HEDIS, 5-Star) and population health goals at point of care, through panel rounding and focused campaigns
  • Utilize our proprietary Electronic Health Record to plan care for individual patients and maximize impact on the population.
  • State of the art practice setting designed for a team-based practice model
  • Our practices offer smaller panel sizes and the opportunity to lead while working with a true team.
  • Lead systemic change by building relationships with outside medical consultants to market the practice model to new patients and stakeholders.

What you’ll need:

  • Board certification in Internal Medicine, Family Medicine, or Geriatrics required.
  • Currently licensed or ability to obtain licensure in the state of Arizona. 
  • Demonstrated leadership experience
  • The ability to build successful relationships with team members and communicate effectively both 1-on-1 and in groups.
  • Can thrive in a fast-growing, mission driven organization focused on using data to improve patient outcomes.
