The Opportunity

 RN Care Manager will be an integral clinical member of our Senior Health Care Teams and play a critical role in value-based care delivery for senior care. The team (providers, health navigators, RN Care Manager, and support staff) collaborates to provide accessible, comprehensive, coordinated care based on longitudinal healing relationships. Our teams provide broad, comprehensive primary care and the RN Care Manager has a key role in ensuring we are successful in this endeavor.

Reporting to the Senior Health Medical Director, RN Care Managers responsibilities center around the delivery of primary care-based clinical nursing care, patient education, and achieving continuous excellence through daily delivery of our unique care.

Full-time (40 hours per week)

What you'll work on:

  • Triage both in office and virtual  patient escalations
  • Perform phlebotomy, IV initiation, and drug therapy management
  • Administer medications and vaccines
  • Perform diagnostic tests (including but not limited to: EKG, ABI, Spirometry)
  • Provide medication reconciliation and adherence
  • Provide treatments and assist providers with procedures and minor surgeries
  • Provide patient teaching, education and support
  • Participate in the clinical development of Health Navigators  
  • Participate in process and quality improvement initiatives 
  • Partners with HSPs to ensure patients continuity of care (co management of care)
  • Monitoring, including lab review and medication adjustment, of anticoagulation patients either in person or virtually 
  • In charge of transitions of care coordinator for patients
  • Oversight of complex care in partnership with health navigators

You’ll need:

  • Active California RN license 
  • 2+ years of nursing experience in hospital or internal medicine or family practice clinical ambulatory setting 
  • Experience preferred in one or more the following: care management, value-based care, care coordination, virtual care
  • Board Certified (NCLEX, BLS)
  • BSN (strongly preferred)

This is a full-time role that will flex between Mill Valley & Greenbrae, CA.
